Privacy notice

This notice explains how Carehorizon Health Private Limited ("Carehorizon Health", "we") uses personal information, including health information, when you contact us or when we coordinate care for you or someone you represent.

1. Who we are

Carehorizon Health Private Limited (CIN U86905WB2025PTC283499) is responsible for the personal information described here. Corporate office: EMCA House, Office 204, Ballard Estate, Mumbai, Maharashtra 400001, India. For any privacy question, request or complaint, write to info@carehorizonhealth.com with the subject "Data protection request".

2. What we collect

  • Contact details: name, phone number, email, and your relationship to the patient.
  • Health information: diagnosis, medical reports, imaging and clinical summaries, which we collect only after first contact and only when needed to coordinate care.
  • Identity and travel documents: passport and visa details, where travel requires them.
  • Information about family members and attendants who travel with or support the patient.
  • Payment information needed to invoice and receive payment.
  • Website information: basic technical data such as browser type and pages visited. We do not use advertising trackers on our enquiry pages.

3. Why we use it

  • To respond to your enquiry and call you back.
  • To coordinate care: sharing records with hospitals and doctors you agree to, arranging transfers, travel, accommodation and visas.
  • To keep you and the people you nominate informed about the case.
  • To invoice and meet our legal, tax and accounting obligations.
  • To send occasional service updates, only if you have opted in.

4. Consent

We use your information on the basis of your consent, given when you submit an enquiry and again, in more detail, before we collect medical records. You may withdraw consent at any time by writing to us. Withdrawal does not affect what was done before, but it may mean we can no longer coordinate the case. If you give us information about another person, you confirm that you have their permission or the authority to act for them. For a patient under 18, consent must come from a parent or lawful guardian.

5. Who we share it with

We share only what is needed, and medical information only with your consent, with: hospitals and doctors involved in the patient's care; aviation, ambulance and medical escort providers involved in a transfer; insurers, assistance companies or employers where they are paying for or managing the case; travel, accommodation and interpreter providers; and the technology providers that host our email and records, under contracts that require them to protect it. We do not sell personal information.

6. International transfers

When we coordinate care across countries, information may be shared with hospitals, providers or insurers outside India. We do this only where needed for the case and as permitted by law.

7. How long we keep it

We keep enquiries that do not become a case for a limited period and then delete them. We keep case records for as long as needed to coordinate care, respond to questions about the case and meet legal obligations, after which they are deleted or anonymised.

8. How we protect it

Access to case information is limited to the people working on the case. Medical records are collected through secure links rather than ordinary email, and our systems use encryption and two-step sign-in.

9. Your rights

You may ask to access, correct, update or erase your personal information, withdraw consent, nominate someone to exercise your rights if you cannot, and raise a grievance. Write to info@carehorizonhealth.com. If you are not satisfied with our response, you may complain to the Data Protection Board of India. Residents of other countries may also have rights under their local law.

10. Changes

We will update this notice when our practices change and show the date of the latest version below.
