Carehorizon Health coordinates inter-hospital transfers, air ambulance cases, medical repatriation and international patient journeys. We work alongside clinical teams and licensed operators, take responsibility for the logistics and communication between them, and document each handover so that everyone involved knows the status of the case.
Hospitals
For international patient departments, transfer desks and admissions teams.
- Inbound patients: complete records before review, visa documents coordinated, and attendants supported so the ward is not managing family logistics.
- Outbound and inter-hospital transfers: receiving-bed confirmation, aircraft and ground ambulance coordination, transfer documents and handover confirmation.
- One point of contact per case, with a named deputy and a defined escalation route.
- Discharge and follow-up: return travel and follow-up appointments coordinated with your team.
Referring doctors
You remain the clinician of record for your patient until care is handed over. We coordinate the transfer or referral on the basis of your clinical summary and recommendations, keep you informed of where the patient is in the process, and return the receiving team's discharge summary to you where the patient consents.
Clinical documents are shared through a secure link after first contact, not through the website form.
Insurers, TPAs and assistance companies
- Coordination of evacuation, repatriation and in-country treatment cases
- Case documents and status reports to your case managers at agreed intervals
- Your cost-approval steps respected: nothing is committed without your authorisation where you are the payer
- Data handled under written data-processing terms
